Current Concept on the Mechanism of the Penile Erection


Abstract
The neurovascular anatomy of the penis is reviewed. Theories regarding the mechanism of human erection are discussed.
The penis is innervated by both divisions of the autonomic nervous system as well as by somatic sensory nerves. The parasympathetic sacral outflow, when stimulated, causes prompt penile erection. The sympathetic thoracolumbar outflow, when stimulated, causes detumescence of the erect peins. The role of the sympathetic nervous system is erection, if any, remains unclear. The role of androgens is primarily through their effect on libido. The vascular mechanism of erection involves increased flow through the pudendal arteries and trapping of blood within the corporal spaces. The neural control of the penile vasculature will require further research to clarify.
